Prescription Transfer
Switching to Queensville Pharmacy? Transferring your prescriptions to us is quick and seamless. Complete our Prescription Transfer Form, or give us a call at (905) 954-4144 and we’ll handle the process, including notifying you when your medications are ready.
If you need medications filled immediately during the transfer, please specify them in the “Questions or Requests” section of the form. Don’t forget to include your date of birth to ensure a smooth process.
Once your transfer is complete, we’ll contact you to confirm, and if requested, let you know when your medications are ready for pickup.
